2 of my recommenders responded promptly and submitted all of their materials by the beginning of Dec. However, the third has not submitted anything and the first set of deadlines was Dec 15th. She last emailed me the week before the deadline that she had set aside time to do everything. However, now the deadline is past and she hasn’t done it.
How do I tactfully approach her about it? I don’t want this to ruin my chance!

There isn’t really any tactful way around it. You simply contact her and say, “Sorry to be so hyper about this, but one of our deadlines is already passed, and I need that information submitted if I am to have a chance at grad school.”
The only other possibility is to find a new recommender, but it’s getting pretty late for that.

My recommendations done the schools you are coming up really soon and then had just wondering when you are coming up really busy right now with exams to mark and then had that sucks what horrible thing to abrasive about it.
